FORM FIVE ADMISSION PROCEDURES – 2025 ACADEMIC YEAR

1. AVAILABLE ACADEMIC COMBINATIONS

Kibo Secondary School offers the following Advanced Level (Form Five) subject combinations:

PCB: Physics, Chemistry, Biology
PCM: Physics, Chemistry, Mathematics
PGM: Physics, Geography, Mathematics
CBG: Chemistry, Biology, Geography
EGM: Economics, Geography, Mathematics
HGE: History, Geography, Economics

2. ELIGIBILITY REQUIREMENTS

To qualify for Form Five admission, applicants must:

A. Academic Qualifications

Attain Division I-III in Form Four National Examinations (CSEE)
Score minimum 10 points in subjects relevant to chosen combination
Demonstrate strong performance in science subjects for science-based combinations

B. Character Requirements

Exhibit good discipline record from previous school
Show commitment to school values: Excellence, Discipline, Hard Work

3. APPLICATION PROCESS

Step 1: Obtain Application Form

- Moshi Bookshop (near Mkombozi Bank)
- Msimbazi Center, Dar es Salaam

Step 2: Complete and Submit

Fill all sections accurately
Attach required documents:
- Original + photocopy of CSEE results slip
- Two (2) recent passport-size photos
- Certified copy of birth certificate
- Bank payment receipt (Tsh. 40,000 non-refundable fee)

Step 3: Interview

Dates: 8th February 2025 OR 1st March 2025

Step 4: Admission Notification

Successful candidates will be contacted via phone/SMS
Receive official admission letter with joining instructions

4. POST-ADMISSION REQUIREMENTS

A. Commitment Fee

small amount of fees will be paid within 2 weeks of admission offer
Failure to pay forfeits admission slot

B. Medical Requirements

Submit medical examination report
Provide vaccination records

C. Uniform & Supplies

Purchase official school uniform
Acquire required textbooks and learning materials

6. SPECIAL NOTES

No late applications will be considered
Original documents must be presented during interview
The school reserves right to revoke admission for:
- Submission of false information
- Disciplinary violations
- Non-payment of fees

Important Application Notice

Please ensure you download both documents. The joining instructions contain crucial information about requirements, deadlines, school regulations, and necessary items to bring. The application form must be completed in full and submitted by the specified deadline with all required attachments.

APPLICATION FEE REQUIREMENT:

All applicants MUST pay a non-refundable application fee of 40,000/= TZS. Failure to include proof of payment with your application will result in immediate rejection. Payment can be made at the school bursar's office or through the designated bank account (details available in the joining instructions).

Applications without complete documentation or proper payment will not be processed under any circumstances.
